Unattached Clinic For Patients Without A Primary Care Provider

The Couchiching Ontario Health Team (COHT) and Couchiching Family Health Team (CFHT) announce the opening of the Couchiching OHT Unattached Clinic. This new clinic, located at the Couchiching Family Health Team (119 Memorial Avenue), is now open to serve patients in the Couchiching region who do not have a primary care provider.

The Couchiching OHT Unattached Clinic is led by a team of Nurse Practitioners offering comprehensive care to individuals of all ages. Patients can either self-refer or be referred by healthcare navigators and other professionals in the community.

The clinic’s primary focus is to provide preventive care, manage chronic conditions, and address acute medical needs – all aimed at preventing chronic disease exacerbations and reducing unnecessary emergency room visits.

  • Diagnosing and treating illnesses and injuries
  • Conducting routine check-ups
  • Ordering procedures and diagnostic tests
  • Referring patients to specialists
  • Prescribing medications
  • Managing chronic health conditions
  • Additionally, the clinic is equipped to support same-day appointments for urgent needs, ensuring timely access to care.

New Orillia Children’s Clinic

The Huronia Nurse Practitioner-Led Clinic (HNPLC) and the Couchiching Ontario Health Team (COHT) are opening a new Children’s Clinic It is made possible with  partnerships in the community including the County of Simcoe Social and Community Services and Orillia Soldiers’ Memorial Hospital. This initiative is designed to provide acute episodic care for children and serve as an essential alternative to Emergency Room visits, offering families a quicker and more efficient healthcare solution for their unwell children and adolescents.
